On-Boarding

Activity one
Parent/Guardian Agreement Form
Activity two
Vanderbilt University Participation Agreement.pdf
This PDF form needs to be filled out and emailed to katrina.g.dubree@vanderbilt.edu
Activity three
Mentee Intake
This intake form is for your child to complete. They may need support, and that's ok! We just ask that their answers are their own.
Training

To streamline the training provided by the EMPOWER VI Mentoring Program, we will list and link each training below in the order we recommend you review them. We estimate the total time you spend on training to be about 60 minutes. Each lesson will have an activity or quiz (opens in a new tab) at the end, and this is how we verify that you have successfully completed the training. Completion of these trainings is required before you attend orientation.
Activity one
Identifying and Protecting Yourself from Abuse (please complete with your child)
Only one end of course survey submission from your child is needed, with your support. You do not need to submit an addtional survey of completion.
Activity two
Microsoft Teams Chat Training and Practice (please complete with your child)
Please note that we ask both you and your child to create a Microsoft Teams account and to complete the end of lesson activity in Microsoft Teams.
Activity three
Mentoring Sessions Overview for Parents and Guardians
This training is meant for parents/guardians to do on their own. Please note there is a Mentee version of this course that we ask your child to complete.
Activity four
Policy and Procedure Manual Guiding Questions (please complete with your child)
We ask for you to support your child to complete the set of questions (one submission is fine).
Activity five
Mentor-Mentee Introduction (following completion of training)
After completing training, Mentees will be notified if a match was made with a Mentor. If a match is made, the EMPOWER VI team will reach out to schedule an introductionary meeting between the Mentee, the Mentor, and the parent or guardian via Teams.
